  4. By using the Patient Portal and the Patient Forms links on our web site you can provide us with all the necessary information to be seen as a patient, saving much of your valuable time in our office!!

Appointments - Rescheduling / Cancellations
If you need to reschedule, call the office during normal business hours and Dial “0” for the operator.

To give all our patients the best possible care, please respect the time we allow for each appointment.

If you need to cancel your appointment we ask that you cancel at least 24 hours prior to the scheduled time.  You must notify our office within 24 hours of your appointment or you will be charged $25.00. This policy also applies to not showing up for your scheduled appointment.

If you must do this after hours please call 631-587-6060 ext 333 and leave your name, date and time of appointment and reason for cancellation.
